Abstract
This study examined the relationship between theory of mind performance, information processing speed, mentalization, and metacognitive beliefs in individuals with Multiple Sclerosis (MS) compared with healthy controls. The sample consisted of 34 patients with MS and 34 healthy controls (N = 68). Participants completed the Reading the Mind in the Eyes Test (RMET), Dokuz Eylül Theory of Mind Scale (DEZTO), Symbol Digit Modalities Test (SDMT), Mentalization Scale (MentS), Metacognitions Questionnaire-30 (MCQ-30), and Beck Depression Inventory (BDI). The MS group demonstrated significantly lower performance in both affective and cognitive theory of mind tasks as well as reduced information processing speed compared to controls. In addition, self-related mentalization and motivation to mentalize were lower in the MS group, whereas pathological metacognitive beliefs related to uncontrollability of thoughts and need for thought control were higher. Regression analyses indicated that information processing speed and cognitive theory of mind were the strongest predictors of group membership. The findings suggest that social cognitive impairments in MS cannot be explained solely by depression or demographic variables and may represent a disease-specific domain of impairment. The study highlights the importance of considering social cognition, mentalization, and metacognitive processes in clinical assessment and rehabilitation planning.
